Flu Immunisation Programme – Monday 14 & Tuesday 15 September 2026
Please complete your child’s consent form
The NHS Immunisation Team will be visiting our school on Monday 14 and Tuesday 15 September 2026 to deliver the annual flu vaccinations to all pupils whose consent has been received.
To ensure your child can take part, you must complete the online consent form (one form per child) as soon as possible using the link below:

Once submitted, you will receive an email confirming your response.
Important Deadline
The consent link will close at midday on Friday 11 September 2026. Please note that the NHS will not be able to accept any consent submissions after this deadline.
The consent form is to be completed by all parents who have children in Reception 2026 up to and including Year 5 students. If your child is currently in Year 6, do not complete the form.
Add the Year group your child is in now (if completing before 1 September), however, if completing after 1st September 2026, add the Year Group that your child will be in from September 2026.
