NHS will be visiting school to deliver the Nasal flu immunisation.
It is very important that you complete the consent form by clicking on the below link, this will take you to the NHS website for you to complete the flu consent form, with whether you consent or decline the flu vaccination.
You must complete the form as soon as possible,.
If you do not respond, the NHS will not have you registered for consent, or not consent, this will cause disruption on the day when the NHS attend and may result in your child not being vaccinated.
Complete the consent form online (one for each child) using the following link, ensure you use the Unique School Code.
You will need to type in the school code, do not copy and paste.
Once you have submitted your online form, you will receive an email confirming receipt.
